- Don’t Forget – Bring a canceled/voided check, or a photocopy of your check, so that we have the routing and account numbers if you want your refund direct deposited. If you owe taxes and want a direct debit, we will also need a cancelled check. We will return the cancelled check with your completed return.
FORM REQUIRING YOUR SIGNATURE – You must bring this form, completed and signed, to your drop off appointment. If you don’t have it, we can’t do your taxes.
If necessary, make an appointment to pick up these documents prior to your drop off appointment by calling the Tax-Aide line – 352-360-6219.
A volunteer will explain the intake process, preparation, quality review, return approval and e-filing.

Scheduling your appointment – 352-360-6219
You must call the Hawthorne Tax-Aide appointment line at 352-360-6219 to make an appointment. If you leave a message, leave your name and phone number. Please speak clearly. A Tax-Aide volunteer will return your call from phone number 352-638-7490. There will be no walk-in appointments.

Performing the Intake Process
Complete the 13614-C Intake & Interview Sheet, sign and bring it with all your tax forms, photo ID and social security cards. You must bring your last income tax return as well as all supporting documents or we will not prepare your return. Both taxpayers must appear at the pickup appointment.
Validating taxpayer authentication
A certified volunteer will validate the photo ID of the person dropping off tax documents and the social security cards or social security income statements for all taxpayers and dependents.
Counselor verification of your documents
A certified volunteer will verify the 13614-C Intake & Interview Sheet is complete and signed by both taxpayers, the 2023 tax return and supporting documents are present and that all of the tax documents for 2024 are present. If anything is missing, the return will not be accepted.
Preparing the tax return
Each return will be prepared by a certified counselor. No documents will be copied and all documents will be placed in a folder clearly marked with the taxpayer’s name and phone number. When the return is completed it will be stored in a locked cabinet until it is reviewed.
Performing the Quality Review
Each return will be reviewed by a second certified counselor. Once the return has been reviewed, it will be stored in a locked cabinet until the assigned pick-up appointment. At that appointment a certified counselor will review the return with the taxpayers. Then taxpayers will sign the required document, take all of their documents and their tax return with them. The return will be e-filed that day by a certified counselor.

Amendments
If we did your taxes and you need to file an amendment, you still need to call the scheduling line at 352-360-6219 to schedule your amendment.
